Accident & Incident Reporting

Any accident or incident, including near misses, as a result of work activities - whether or not there are apparent injuries or ill health arising, should be reported to Safety Services.

Accidents/incidents can be reported using Inside UCL app (https://app.ucl.ac.uk/InsideUCL/Home).

 These incidents will be investigated by the DSO and involve appropriate managers where appropriate. The information is used to identify and analyse hazards and risk which, in turn, is used to protect people by helping to prevent further or more serious incidents.

 The report form must be completed as soon as possible following an incident (note: anyone may report an incident, not just the injured or concerned party).

Serious accidents must be reported immediately, directly to the Departmental Safety officer (X33056) and to Safety Services (X46944). As required by law, serious accidents or incidents will be reported by Safety Services to the Health and Safety Executive (HSE) and/or other relevant regulatory bodies.
